Walking into a chiropractor's office for the first time can feel a bit intimidating, especially if you've never experienced chiropractic care before. You might be wondering: Will it hurt? What will they do? How long will it take? What should I wear?
If you've been putting off scheduling your first appointment because you're unsure what to expect, this guide is for you. We'll walk you through everything that happens during a typical first chiropractic visit so you can arrive feeling confident and prepared.

Before Your Appointment: What to Do
Scheduling Your Visit: When you call to schedule, we will ask about your main concerns and send you the new patient paperwork. Our office offers online forms that you can complete from home, saving you time on your appointment day. Plan for your first visit to take about 75 minutes, which is longer than follow-up appointments.
What to Bring: If you plan to bill your insurance, please bring your insurance card and a photo ID. It's also helpful to bring a list of current medications and supplements, any recent X-ray or MRI results related to your condition, and a list of questions you would like to ask. If you have specific health concerns or a detailed medical history, writing down key points beforehand can be super helpful.
What to Wear: Wear comfortable, loose-fitting clothing. You won't need to change into a gown, but you want to be able to move freely. Avoid clothing with a lot of zippers, buttons, or thick seams along the spine. Many patients wear athletic wear or casual clothes. Women may want to avoid dresses or opt for leggings underneath.
Step-by-Step: Your First Appointment
1. Welcome and Paperwork (10 minutes)
When you arrive, you'll be greeted by our staff. If you haven't completed your paperwork online, you'll fill out forms covering your medical history, current symptoms, past injuries or surgeries, family health history, lifestyle factors like work, exercise, and stress levels, and your health goals and what you hope to achieve with chiropractic care.
This paperwork isn't just bureaucracy. It helps us understand your complete health picture so we can provide the safest, most effective care possible.
2. Consultation with Your Chiropractor (20-30 minutes)
Next, you'll meet with your chiropractor for a one-on-one consultation. This is your chance to explain what brought you in. Your chiropractor will ask detailed questions about your pain or discomfort, including when it started, what makes it better or worse, whether it radiates or stays in one place, and how it affects your daily life and activities.
This is also the perfect time to ask any questions you have about chiropractic care, share any concerns or fears, and discuss your health goals beyond just pain relief. Don't hold back. There are no silly questions or anything that is TMI (too much information). We want you to feel completely comfortable, supported, and understood.

3. Physical Examination (15-20 minutes)
The physical exam is thorough and may include several components. Your chiropractor will assess your posture and how you stand and move, evaluate your spine's range of motion, perform orthopedic tests to identify specific problem areas, check for muscle tension, weakness, or imbalances, and conduct neurological tests like reflexes and sensation. They may also palpate (feel) your spine to identify areas of restriction, misalignment, or tenderness.
Will the exam hurt? The exam should not be painful, though certain movements or palpation might reproduce your symptoms. This is actually helpful information that guides your treatment. Always let your chiropractor know if something hurts.
4. Diagnostic Imaging (If Needed)
Depending on your condition, your chiropractor may recommend X-rays or other imaging. Not every patient needs imaging, but it may be recommended if you have a history of trauma or injury, symptoms that suggest something more serious, unexplained pain, conditions that require visual confirmation, or if you're over a certain age and haven't had recent imaging. While we do not have any X-ray equipment on site, we work with a few trusted imaging centers in Chicago to get you the imaging you need.
If any imaging is performed, your chiropractor will review the results with you and explain what they show.
5. Report of Findings and Treatment Plan
After gathering all this information, your chiropractor will explain what they found, what's causing your pain or dysfunction, and how chiropractic care can help. They'll present a treatment plan that includes the type of treatment recommended, how often you'll need to come in initially, and the expected timeline for improvement. They'll also discuss the costs involved and answer any questions about your care plan.
This is a conversation, not a lecture. A good chiropractor will make sure you understand everything and feel comfortable moving forward. You should never feel pressured to commit to a long treatment plan on the spot.
6. Your First Adjustment
Some chiropractors will perform an adjustment during your first visit, while others prefer to wait until the second appointment after reviewing X-rays or having more time to plan your care. There's no right or wrong approach, and it depends on your specific situation.
If you do receive an adjustment on your first visit, here's what to expect:
Before the Adjustment: Your chiropractor will explain exactly what they're going to do, position you on a specialized chiropractic table, and may use a drop table, activator tool, or manual adjustment technique.
During the Adjustment: You may hear a popping or cracking sound. This is just gas bubbles releasing from the joint and is completely normal. The adjustment itself is typically quick, just a few seconds per area. Most patients describe it as pressure followed by relief, not pain.
After the Adjustment: You might feel immediate relief, soreness similar to after a workout, increased range of motion, or relaxation and even tiredness. All of these responses are normal.
What Happens After Your First Visit?
Immediate Aftercare: Your chiropractor will give you specific instructions, which might include applying ice or heat, avoiding certain activities for 24 hours, drinking plenty of water, or doing gentle stretches. They'll also schedule your follow-up appointments based on your treatment plan.
How You Might Feel: Everyone responds differently to their first adjustment. Some people feel amazing immediately with reduced pain and improved mobility. Others experience mild soreness for a day or two, similar to post-workout soreness. Some feel tired as their body begins healing. Rarely, some people experience a brief increase in symptoms before improvement, which is called a healing reaction.
All of these responses are normal. Your body is adjusting to changes in alignment and function. However, you should contact your chiropractor if you experience severe pain, new symptoms, or symptoms that don't improve within 24-48 hours.
Common Questions About First Visits
"Will my first visit include treatment, or is it just a consultation?" This varies by practice and your specific situation. Many chiropractors will provide treatment on the first visit if appropriate. Always ask when scheduling to find out upfront.
"How much will my first visit cost?" First visits typically cost more than follow-up visits because they include the comprehensive exam and consultation. With insurance, the amount you may be responsible for can be variable, depending on your specific plan. Call ahead to get specific pricing.
"Do I need a referral from my doctor?" In most states, you don't need a referral to see a chiropractor. However, some insurance plans do require one, so check with your insurance company if you're planning to use benefits.
"Can I eat before my appointment?" Yes, please eat normally. You don't need to fast or prepare in any special way.
"What if I'm nervous about the adjustment?" Tell your chiropractor! They can use gentler techniques, explain everything in more detail, or even wait until a future visit when you feel more comfortable. Good chiropractors never force treatment.
"Will I have to keep coming back forever?" You're in control of your care. While some patients choose ongoing maintenance care, there's no obligation. Your chiropractor will recommend what they think is best, but the decision is always yours.
Red Flags: What a First Visit Should NOT Include
A reputable chiropractor will never pressure you to commit to a long, expensive treatment plan immediately, require you to pay for months of treatment upfront, dismiss your concerns or questions, refuse to explain their findings or treatment plan clearly, or guarantee specific results or promise to cure serious diseases.
If you experience any of these red flags, trust your instincts and seek a second opinion.
Tips for Getting the Most from Your First Visit
To maximize your first appointment, be honest and thorough when describing your symptoms and health history. Don't downplay pain or symptoms because you think they're not related. Ask questions about anything you don't understand. Take notes or bring someone with you if you think you'll forget information. Be open to the process, but also trust your gut. Arrive a few minutes early to avoid feeling rushed. Turn off your phone and be present during the consultation.
